New to our office? You're in the right place! This page will (hopefully) answer all your questions and give you a better understanding of what to expect when you come in for your first visit!
All of our new patient paperwork is electronic and designed to save you time and unnecessary headaches! Simply provide us with a valid email address when scheduling your visit and you will receive a link to complete the paperwork prior to your scheduled appointment. Everything is submitted securely and electronically, so there is no need to print anything.
We do require that paperwork be completed at least 24 hours prior to your scheduled visit to allow the provider to review your medical history. If you are unable to complete your paperwork ahead of time, please let our front desk staff know.
As a courtesy to our patients, we will gladly bill your insurance company for medically-necessary chiropractic treatment. We are contracted with several major insurance companies and will do our best to verify your insurance benefits prior to beginning treamtent. Please note, insurance benefits are an agreement between you (the patient) and the insurance company. All verification of benefits are an estimate of coverage and are not a guarantee of payment. Should your insurance company deny coverage, you will be financially responsible for all charges incurred in our office.
PLEASE BE ADVISED: Massage Therapy is not typically a covered insurance benefit,
therefore we do not bill medical insurance for massage therapy services.

As an out-of-network provider for the above insurance companies, we are not contracted directly with the insurance company. We are happy to provide you with an itemized superbill for you to submit directly to your insurance, however payment in full is expected at the time of service. If coverage exists, the insurance company will reimburse you directly.
MEDICARE PATIENTS: We are a participating provider for Medicare Part B and all supplemental plans. Please note, Medicare coverage is limited to manipulation of the spine for medically-necessary treatment and does not cover the initial exam fee or maintenance/wellness care. If you have questions or concerns about your coverage, please give us a call and we will happily review your benefits.
